Welcome to Person Directed Supports where our goal is to provide essential resources and supports in a loving, caring environment to individuals with intellectual disabilities. Founded in February of 2000, PDS is proudly a family owned and operated not for profit organization.

Our purpose is to support each individual using “Person-Centered Planning.”

What is Person-Centered Planning?

Person-Centered Planning (PCP) is a set of approaches designed to assist someone to plan their life and supports. It is used most often as a life planning model to enable individuals with disabilities or requiring support to increase their personal self-determination and improve their own independence. Person Centered Planning is used in Community Homes, Life Sharing, Behavior and Community Supports Programs and more to increase proactivity and a fulfilling life.

 

Our Mission

The mission of Person Directed Supports is to provide an opportunity for people and their families to learn about a larger world and to make decisions about how they choose to experience and contribute to the resources, joys and traditions of the community.  Through a person centered planning process, comprehensive supports coordination, and best practice treatment and habilitative approaches; people will learn, grow and enjoy a wide array of new experiences, relationships and activity.
